Website Privacy Policy (and Privacy Policy Changes)

This privacy policy pertains to use of the website of the Momenta Clinic (the “Site”) and any mobile or web applications developed or used by Momenta Clinic (the “Applications”) and is separate from our clinical Privacy Policy which is submitted to patients for review and consent prior to the delivery of clinical services from Momenta Clinic, including but not limited to, therapy, assessment and consultation. Momenta Clinic is a division of the Chattha Psychological Professional Corporation. Please carefully review the following Site Privacy Policy concerning any information we may collect while operating our Site. By using this Site, you agree to the terms in our Site Privacy Policy in full. If you do not accept all the terms of this Site Privacy Policy, you must not use the Site.

Momenta Clinic reserves the right to change terms in the Site Privacy Policy from time to time at its sole discretion. It is therefore important that you review the Site Privacy Policy regularly to ensure you are updated as to any changes. Accepting the terms of the Site Privacy Policy applies to all visits to the Site, both now and in the future as well as use of the Applications.

Momenta Clinic uses and discloses identifying information to employees, contractors, affiliated organizations on a “need to know” basis, which may include, but is not limited to, the purpose of providing services or answering queries. Our employees, contractors, students, volunteers and other personnel who may engage with identifying information from the Site are bound to our privacy policy and confidentiality agreements. Some of these persons may be located outside Canada and by use of our Site, you consent to the transfer of such information to them. Momenta Clinic or its personnel will not sell your information to anyone. We are subject to disclosure of identifying information under the terms of our Clinical Privacy Policy, which may include abuse/neglect of a minor, risk of imminent harm to a person or persons, court subpoena or order, learning of abuse by a regulated health care practitioner, disclosure required by law or review by regulatory bodies.
